Practice catchment area
The practice area is shown with our catchment area marked in blue. Go to the practice catchment area page and use the map and enter your postcode to check if you live within our catchment area.
If you are unsure, you can also contact reception to confirm.
If you change your name and address or telephone number please inform us as soon as possible. If you move out of the practice area, you will be required to re-register with a new practice local to your new address.
How to register with the practice
If you live in our practice catchment area you can register with the practice.
Once you have confirmed that you live within our catchment area you can use the online registration forms. Visit gp-registration.nhs.uk/G85721 or download a paper version of this form [EXTERNAL PDF LINK] and hand and return your completed registration forms to reception or email to souccg.301eaststreetsurgery@nhs.net.
Registrations are usually processed within 3 to 5 working days but could take longer if we need to check anything with you from the forms you have provided.

Online services
We recommend that you use the NHS App or accurx which allows you to seek medical advice, book appointment, view aspects of your medical record such as results from tests, consultations as well as order repeat medication.

If you do not have a smartphone or tablet device but still want access to online services, please email us or ask at reception access to online services. We will ask for photo ID (Driving Licence or Passport) as proof.
Summary Care Record
A Summary Care Record is an electronic record containing key health information, which can be made available to NHS healthcare staff caring for you in an emergency or when your GP practice is closed.
Why do I need a Summary Care Record?
Storing information in one place makes it easier for healthcare staff to treat you in an emergency, or when your GP practice is closed.
This information could make a difference to how a doctor decides to care for you, for example which medicines they choose to prescribe for you.
Who can see it?
Only healthcare staff involved in your care can see your Summary Care Record.
Do I have to have one?
No, it is not compulsory. If you choose to opt out of the scheme, then you will need to complete a form and bring it along to the surgery. You can use the form at the foot of this page.
